Title:Assessment of anti-PGL-I as a prognostic marker of leprosy reaction.

Abstract:The anti-phenolic glycolipid-I (PGL-I) assay as currently applied for leprosy is conceived as an early marker of asymptomatic infection, early disease diagnosis and cure monitoring. Its use as a prognostic marker of reaction is still a matter of controversy. We conducted a case-control study to investigate whether IgM and IgG anti-PGL-I antibodies could discriminate patients at increased risk of developing reactions. Eligible cases were untreated leprosy patients at the onset of type 1 and type 2 reactions recruited from among 600 concurrent, newly detected, untreated leprosy patients attending an outpatient clinic in central Brazil. For the patients with reaction, approximately the same number of leprosy cases without reaction matched as to bacterial index (BI), age and gender were randomly selected. Individuals without clinical leprosy were evaluated as healthy controls. Sera from type 1 reaction (N = 43) and type 2 reaction (N = 26) patients were tested by an ELISA using PGL-I synthetic disaccharide-BSA antigen and 1:300 sera dilution (cut-off point > or = 0.2 OD). Antibody profiles were evaluated by exploratory data analysis and reverse cumulative distribution curves. The IgG anti-PGL-I response did not have a defined pattern, being detected only at low levels. Our results indicate that leprosy patients, independently of their reactional status, produce high levels of IgM anti-PGL-I, demonstrating a strong correlation between the magnitude of antibody response and the BI. Patients with a higher BI were at least 3.4 times more prone to produce an antibody response compared to healthy controls. (AU)^ien.

Title:Specific anti M. Leprae PHL-1 antibodies and mitsuda reactions in the management of household contacts in New Caledonia.

Abstract:Household contacts of leprosy patients have been tested for anti-phenolic glycolipid-I IgM antibodies (anti-PGL-I IgM) by an ELISA using the natural disaccharide (NDO) and natural trisaccharide (NTO) synthetic antigens. A group of healthy subjects without known exposure to Mycobacterium leprae served as controls. The percentages of positivity observed in multibacillary patients, paucibacillary patients, and household contacts were significantly higher than those of the negative controls. The absorbance values using NDO and NTO correlated well (range 0.59-0.91) when analysis of each subject group was performed. As reported here, NDO and NTO antigens seem to be equal in detecting leprosy cases; 100% of multibacillary and 21.43% of paucibacillary cases were detected as seropositive. For the screening of household contacts, NDO appears to be more sensitive and NTO more specific. There were more seropositive cases in the young age groups of household contacts, suggesting a higher rate of transmission of M. leprae infection in those age groups. Lepromin and anti-PGL-I IgG tests were also performed in contacts who were followed. The 2 paucibacillary subjects (1 borderline tuberculoid, 1 indeterminate) were Mitsuda negative. At diagnosis, their anti-PGL-I IgM levels were much higher than those of previous results; their anti-PGL-I IgG levels showed an increase in one and a decrease in the other. However, for the entire group anti-PGL-I IgM and anti-PGL-I IgG levels were positively correlated. The data reported here suggest that an increase in specific anti-M. leprae IgM levels in Mitsuda-negative household contacts could reveal the development of overt disease.

Title:Quantitative antibody ELISA for leprosy.

Abstract:Quantitative enzyme-linked immunosorbent assays (ELISAs) were established to measure IgM and IgG antibody levels to soluble Mycobacterium leprae sonicate (CD60) and to the synthetic disaccharide antigen based on the phenolic glycolipid-I antigen of M. leprae coupled to bovine serum albumin in 46 leprosy patients. Separate reference pools for IgM and IgG antibody were established. The reciprocal of the antibody titer was expressed as the number of arbitrary units in the reference pools which was subsequently used as the calibrator for assessment of units in individual test sera. The dose-response relationship for both IgM and IgG was highly specific and reproducible for both isotypes, as indicated by the intra- and inter-assay coefficients of variation. The distribution of antibody levels are in general agreement with the results from previous studies against different M. leprae antigens. The lepromatous group showed 10- to 100-fold higher IgM antibodies to both the soluble sonicate antigen and the disaccharide as compared to the control group. Very low to undetectable levels of IgM antibodies were observed in the tuberculoid group of leprosy patients. IgG antibodies, on the other hand, were not only present but showed considerable overlap with the lepromatous patient group. Optimized ELISAs, such as the one described in this study, would allow one to address issues such as antibody changes with treatment, antigen clearance, and correlation with other immune parameters associated with disease pathogenesis and protection.

Abstract:Os testes sorologicos para diagnostico de hanseniase, usando o glicolipideo-fenolico-1 (PGL-I), considerando antigeno especifico do M. leprae, tem aberto algumas possibilidades de estudo do comportamento epidemiologico desta doença. Algumas questoes, como tempo de latencia da doença, infecçao subiclinica e importancia do contato intra-domiciliar (contatos) no controle da endemia puderam ser melhor analisadas usando este instrumental. Este estudo teve por objetivo verificar a existencia de associaçao entre a situaçao sorologica e a ocorrencia de hanseniase. Foram seguidas durante 4 anos, 6.520 pessoas com idade igual ou superior a 5 anos, submetidas no inicio do seguimento ao teste sorologico Anti PGL-1, pertencentes ao universo de 7.416 habitantes da area urbana de um municipio paulista caracterizado por elevada endemicidade de hanseniase. Foi identificado um grupo de 590 individuos soropositivos (9.0%). Foram diagnosticados, no periodo, 82 casos novos de hanseniase, 26 no grupo de soropositivos (441 casos novos/10.000 individuos) e 48 no de soronegativos (81/10.000). Entre os que nao fizeram sorologia, surgiram 8 casos novos (89/10.000). Procurou-se controlar, na analise, a condiçao de contato, dado que a taxa de soropositivo padronizada por idade e sexo era de 9.61% no grupo de contatos e 7.65% no de nao-contatos. Tomando-se aos de nao-contatos soronegativos como o grupo de "nao espostos", foram calculados os riscos relativos de adoecimento no periodo, a partir das taxas de detecçao padronizadas por idade, resultando no seguinte: os contatos ID soropositivos apresentaram a taxa de 1.704/10.000, 27 vezes maior que a dos "nao-expostos", igual a 63/10.000, os nao-contatos soropositivos e os contatos soronegativos apresentaram taxas, respectivamente, de 274 e 198/10.000 ambas maiores que as dos "nao-expostos" e iguais entre si. A soropositividade associou-se a elevaçao de 8,6 vezes do risco de hanseniase entre os contatos de 4,4 entre os nao-contatos. Na situaçao epidemiologica estudada, caracterizada por elevada endemicidade de hanseniase, 50% dos casos novos surgiram entre os nao-contatos soronegativos, ou seja, sem fonte de infecçao conhecida. Portanto o test anti-PGL-1 usado revela-se na pratica, de pouca aplicabilidade. Resta estudar ainda o comportamento da sorologia anti-PGL-1 em areas de media e baixa endemicidade par que se possa tirar conclusoes mais consubstanciadas sobre sua utilidade no controle da endemia (AU).

Abstract:Analisou-se a prevalencia de anticoepos IgM utilizando teste sorologico e antigeno especifico Glicolipidio Fenolico I (PGL-I) do Mycobacterium leprae, por meio do teste rapido ML Dipstick em 1611 individuos sadios categorizados em contatos de pacientes hansenicos intradomiciliares e comunitarios, residentes em cinco municipios endemicos para hanseniase. Realizou-se estudo tranversal pela analise do banco de dados do laboratorio de hanseniase, Instituto Evandro Chagas. O estudo estendeu-se de março a dezembro de 1998, com reavaliaçoes anuais ate dezembro de 2002, no qual, os municipios selecionados-Curinopolis, Eldorado do Carajas, Xinguara, Rondon do Para e Dom Eliseu - apresentaram taxas de prevalencia em hanseniase maiores que 20 em cada 10.000 hab. e detecçao de casos novos maiores que quatro em cada 10.000 hab. A idade variou de 2 a 90 anos, devido ao fato de que a doença em areas de alta endemicidade possibilita o diagnostico de crianças. Estudou-se 957 mulheres (957/1611-59.4 por cento), 654 homens (654/1611-40.6 por cento) e observou-se predominancia de soropositividade entre as mulheres na faixa etaria entre 21 e 50 anos, 62.81 por cento contra 53.97 por cento nos homens. A analise foi realizada nos bancos de dados D-BASE, EPI-INFO 6.4 e 2002, BIOESTAT 3.0 e SINAM, existentes no Instituto Evandro Chagas e Secretaria Estadual de Saude do Para...(AU).

Abstract:Forty biopsies from skin lesions of untreated (24) and treated (16) leprosy patients, representing the whole leprosy spectrum, were examined by means of immunofluorescence (IF) methods. Only few or no cells staining with FTC-labelled antihuman immunoglobulin antisera were found in the lesional skin of all patients examined. Sparse IgM-deposits along the basement membrane of the epidermis were observed in a few borderline lepromatous and lepromatous patients. Complement deposits along the basement membrane of the epidermis and in the vessel walls were found in tuberculoïd as well as in lepromatous patients. Complement and in a lesser number IgG-deposits were observed around sweat glands and sometimes around sweat gland ducts and other skin appendages. Autofluorescing macrophages were noted in patients at the lepromatous side of the leprosy spectrum; approximately half of these patients showed complement deposits in an around these cells.(AU).

Abstract:Immunofluorescent banding of the dermal-epidermal junction of skin was demonstrated in 3 of 7 patients with lepromatous leprosy by direct immunofluorescence microscopy. The banding was caused specifically by deposition of IgM. Within the glomeruli of one patient, dense, amorphous deposits in subendothelial and intramembranous position were also demonstrated by electron microscopy. These preliminary findings suggest that lepromatous leprosy may be associated with immunologic disurbances of both skin and glomerular basement membranes.(AU).

Abstract:A report is given of the use of the enzyme-linked immunosorbent assay to measure antibody to preparations of human thymocyte membranes (HTMA) and to beta 2-microglobulin. The assay described is simple and rapid, and requires only small quantities of an easily stored membrane preparation. The advantages of this technique over conventional methods involving cytotoxicity are discussed. Raised levels of IgM antibody to beta 2-microglobulin were detected in sera from SLE patients. Raised levels of IgG and IgM antibody to HTMA were found in sera from most active lepromatous cases. Two of eight sera from SLE patients showed raised IgG anti-HMTA, but not raised IgM. An attempt was made to study the subclass of the IgG antibodies found, but when checked against purified human IgG myeloma proteins, the available anti-subclass sera were found to lack the necessary degree of specificity in this assay. Abstract:For the first time in Brazil it was investigated the occurrence of IgM anti-PGL-1 in the sera of household contacts of leprosy patients using the ELISA methodology. The sera of the multibacillary patients showed significantly more immuno-reactivity than from the paucibacillary patients. It was observed a high subclinical infection incidence among household contacts (19.4%). The percentage of leprosy development was 5% (1/21) among the seropositive contact group. This finding suggests that serology could be useful as prognostic test, but for better definition is necessary to test a population from endemic area for long period time.(AU).
